Empowering customers to live rewarding and happy lives. That’s when it hits home.
Due to the expansion of our team, we have a fantastic job opportunity for an enthusiastic Support Coordinator like you, to join our Central Resource Team (CRT).
Here in Teesside, Durham and Sunderland Home Group have a large number of support services, which help our customers who have individual and varying needs.
Instead of working in one service, our CRT gives you the opportunity to work across a wide range of supported services and alongside our specialised teams, helping to enrich our customers lives.
As our Support Coordinator, you’ll help change our customer’s lives for the better by undertaking risk assessments, support planning, goal setting and regular reviews.
You’ll also create individual and personalised support plans with and for customers. Each plan will look to meet the customer’s individual needs and aspirations, making sure they meet safeguarding and confidentiality requirements.
You’ll also have responsibility for coordinating our support workers and working collaboratively with external agencies to assess and engage support for our customers.
Your future career with Home Group could start with our Central Resource Team. If you like variation in your working life, meeting lots of different people and supporting individuals then the CRT is definitely for you! No two days are the same and are always rewarding.
At CRT we like to recognise our differences and celebrate them and we encourage you to be your true authentic self.

You’ll promote independence and ensure the dignity, privacy and the rights of our customers by supporting them in a non-discriminatory person-centred way.
As our Support Coordinator, you’re flexible, resilient, and adaptable in your approach as you’ll work where the business needs your support.
It would be amazing if you have has a job in this type of environment before, however it’s your “can do” positive approach and enthusiasm that are critical to making this role a success. We can nurture your talent, just let us know in your cover letter why you have a passion for supporting people.
To make sure that we keep our records straight, and customers safe, it's necessary that you have good IT skills or are enthusiastic to learn.
Finally, as you’ll travel around the area to work in our services, it is essential that you have access to a vehicle (we pay your expenses to cover this use).
When you join, you’ll need to have a valid Enhanced Check under the Disclosure and Barring Service. The great news is that we will pay for that to be done if you don’t have a transferrable one.
We are an outgoing, diverse and supportive group of individuals, always willing to lend a helping hand.
Our team is led by our amazing Senior Client Service Manager, Yvonne who has worked for Home Group since 2014. Yvonne is perfectly placed to support you as you join. She started as a Support Worker and took full advantage of all the training and experience we offer here at Home Group.
Supporting Yvonne are our two wonderful Client Service Managers Benjiee and Shannon who oversee the team on a daily basis. They started their Home Group journeys with the Central Resourcing Team and took the opportunities available to progress.
